• The Patient must be under the care of a physician.

• The patient must need intermittent skilled care. (Skilled nursing, PT,OT,ST, etc.)

• The patient must live within the agency's geographical service area.

• The patient must meet all insurer requirements for care. (example below)

For Medicare Patients

• Patient must be homebound.

• Patient will need a face to face encounter with a physician, nurse practitioner, clinical nurse specialist, physician's assistant, for a matter related to the patient's need for home health services within 90 days before start of care or within 30 days after start of care.
